Weekly pay ranges from $2,187 to $2,303, with guaranteed hours. EMR: Epic; population predominantly 65+.

Travel assignment with agency support from Healthforce Healthcare LLC. Requires MSW and licensure pursuit toward LCSW, 2+ years of relevant experience, active driver's license and BLS.

Qualifications

• Master’s in Social Work (MSW) is required.
• Actively pursuing licensure toward LCSW in the appropriate jurisdiction.
• Minimum 2 years of clinical/ED/Medical Social Worker experience.
• Active driver’s license with reliable vehicle and BLS certification.
• Proficiency with Epic EMR; ability to perform biopsychosocial assessments and documentation.
• Comfort working with a population predominantly 65+.

Responsibilities

• Conduct comprehensive biopsychosocial assessments for patients and families in hospice care and document findings in Epic EMR.
• Provide psychosocial support, counseling, and care coordination; participate in interdisciplinary team meetings to align goals of care and discharge planning.
• Facilitate advance directives, goals-of-care discussions, and family meetings; connect patients and families with appropriate community resources and services.
• Assess safety, functional status, and psychosocial needs; assist with planning and coordination of post-acute or home-based support; document lift requirements up to 50 lbs intermittently.
• Maintain HIPAA compliance and collaborate with the team to optimize patient outcomes.
• Travel within the defined service area with a reliable vehicle; hold an active driver’s license and BLS certification.
