Job summary

Sweetwater Care is seeking a licensed LVN in California to provide nursing care in a long-term care setting. You will follow physician orders, policies, and participate in quality improvement and patient education.

Responsibilities include timely administration of medications and treatments, transcribing orders, and assisting physicians during rounds as needed, with accurate charting and family communication.

Qualifications

• Currently licensed LVN in California; compact license if working in a compact state.
• One-year nursing management experience preferred.
• Long Term Care experience preferred.
• Paper charting and PointClickCare charting experience are preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Monitors patients using the Quality Life Manual.
• Notifies departments of patient changes.
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.
