Job summary

Doctor's Choice Home Care & Hospice Texas is seeking an LVN to administer skilled nursing care in patients' homes. You will work under the supervision of a Registered Nurse, following physician orders and the plan of care.

The role requires travel to patient locations, effective communication with families, and participation in ongoing education and QAPI activities. A valid TX LVN license and CPR certification are required.

Qualifications

• Must be currently licensed in the state(s) practicing
• Minimum 1 year nursing experience (preferred)
• CPR certified (American Heart Association or Red Cross)
• Ability to exercise initiative and independent judgment
• Excellent written and oral communication skills
• Strong customer relations and commitment to quality service
• 100% travel may be required
• Valid driver’s license and automobile liability insurance

Responsibilities

• Provide skilled nursing care per physician orders in patients’ homes
• Collaborate with RN and other health practitioners on care plans
• Educate patients, caregivers, and families
• Document patient status and interventions for QAPI program
