Job summary

ConnectedHealth is seeking a Travel Long-Term Care RN for a 13-week assignment to provide skilled nursing care to LTC residents across facilities in Montana. You will collaborate with interdisciplinary teams, assess patients, implement care plans, and document progress in the EMR system.

The role requires an active Montana RN license, BLS, and at least 2 years of LTC experience with strong wound care skills.

Qualifications

• RN license in Montana required.
• BLS certification required.
• Minimum 2 years of long-term care experience.
• Strong wound care experience.
• Experience with LTC patient management and care planning.
• Ability to work scheduled days including holidays and weekends.
• Good communication and teamwork skills.

Responsibilities

• Provide skilled nursing care to LTC residents in accordance with facility policies and state regulations.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Document patient assessments, treatments, and progress accurately in the EMR system.
• Administer wound care treatments, medications, and other nursing interventions as prescribed.
• Maintain effective communication with patients, families, and team members to ensure high-quality care.
