Job summary

HealthForce Healthcare in Vermont is seeking an experienced Travel Long Term Care LPN to provide high-quality nursing care in skilled nursing and long-term care settings. You will administer meds, monitor vitals, and support residents with daily living activities while following care plans.

This full-time contract travel position requires an active VT LPN license (or multi-state compact), BLS/CPR, and 1+ year LTC/SNF experience. Travel stipends and housing allowances are offered.

Qualifications

• Graduate of an accredited practical nursing program.
• Active Vermont LPN license (or multi-state Compact).
• Current BLS/CPR certification (AHA preferred).
• 1+ year of recent LTC/SNF experience.
• Strong communication and documentation skills.

Responsibilities

• Administer medications and treatments; monitor resident responses.
• Record vital signs and report changes to RNs/physicians.
• Perform wound care and routine procedures per care plans.
• Maintain thorough EHR documentation and charting.
• Collaborate with CNAs, therapists, physicians and families.
• Assist with ADLs and rehabilitation goals, adhering to safety.
