HealthForce Healthcare in Vermont seeks an experienced Travel Long Term Care LPN to provide person-centered care in skilled nursing and long-term care settings. You will perform routine assessments, manage chronic conditions, and coordinate care with an interdisciplinary team in a fast-paced environment.

The role requires a current LPN license (Vermont or compact), BLS/CPR, and 1+ year LTC/SNF experience. Travel nursing, flexible shifts, and housing stipends are available.

Qualifications

• Graduate of an accredited practical nursing program.
• Active LPN license in Vermont or a valid multi-state Compact license.
• Current BLS/CPR certification (AHA preferred).
• Minimum 1+ year experience in LTC/SNF.
• Strong interpersonal and clinical communication skills; proficiency with EHR documentation.
• Open and willing to work all shifts, including weekends and holidays.

Responsabilités

• Administer scheduled medications, treatments, and monitor resident responses.
• Monitor and record vital signs; report acute changes to RNs/physicians.
• Perform wound care and routine clinical procedures per care plans.
• Document thoroughly in the facility’s EHR system.
• Collaborate with CNAs, therapists, physicians, and families to foster compassionate care.
• Support residents with ADLs and rehabilitation goals, ensuring safety and infection control.
