Job summary

HealthForce Healthcare in Vermont seeks an experienced Travel Long Term Care LPN to deliver person-centered care in skilled nursing and LTC facilities. You will perform routine assessments, administer meds, and support residents' quality of life while traveling across assignments.

The role requires an active Vermont LPN license, BLS/CPR, and at least 1 year in LTC/SNF. Travel stipends and housing allowances are provided alongside 24/7 recruiter support.

Qualifications

• Graduate of an accredited practical nursing program.
• Active Vermont LPN license (or multi-state Compact).
• Current BLS/CPR certification (AHA preferred).
• Minimum 1+ year LTC/SNF experience.
• Strong interpersonal and clinical communication skills; proficient with EHR documentation.
• Open and willing to work all shifts, including weekends and holidays.

Responsibilities

• Administer scheduled medications, treatments, and monitor resident responses.
• Monitor and record vital signs; assess and report acute changes to RN or physician.
• Perform wound care, dressing changes, and routine clinical procedures.
• Maintain clear, thorough documentation in the EHR system.
• Collaborate with CNAs, therapists, physicians, and resident families to foster a compassionate care environment.
• Support residents with ADLs and rehab goals while ensuring safety and infection control.
