Job summary

Healthforce is seeking a Licensed Practical Nurse (LPN) for a travel—temporary assignment in Wallingford, VT. The role requires an active Vermont LPN license, the ability to work overnight shifts, and travel readiness.

It spans outpatient and inpatient rehab settings with guaranteed 40 hours per week and weekly pay. Responsibilities include direct patient care, medication administration, vital signs monitoring, and teamwork with RNs and multidisciplinary staff to support recovery goals.

Qualifications

• Active Vermont LPN license.
• Ability to work the overnight shift.
• Willingness to travel for a temporary assignment.
• Experience in long-term care or rehabilitation settings preferred.

Responsibilities

• Provide direct patient care for adults in long-term care and rehab.
• Administer medications and document administration per policy.
• Monitor vital signs and respond to changes in condition.
• Assist with care planning, documentation, and routine nursing tasks under supervision.
• Collaborate with physicians, therapists, counselors, and other professionals to support safety and recovery goals.
• Maintain infection control practices and accurate charting in EHRs.
