Job summary

Sweetwater Care in Nashville is seeking an LVN/LPN to provide nursing care within scope of practice, following physician orders, center policies, and applicable guidelines. The role requires long‑term care experience and a valid LVN/LPN license in the practicing state, plus current BLS/CPR certification or willingness to obtain it.

You will support charting in PointClickCare, document patient condition changes, and participate in rounds with physicians.

Qualifications

• Long-term care experience required.
• Experience with PointClickCare charting.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight loss, or other changes.
• Uses the Quality Life Manual for identification, placement, and monitoring of patients in appropriate programs.
• Documents room changes, LOA, new admits, or discharges and notifies applicable departments.
• Makes rounds with physicians when necessary.
