Job summary

American Traveler is seeking a licensed practical nurse for a travel assignment in Middlebury, Vermont. You will join a post-acute skilled nursing rehab unit, providing skilled nursing care to patients aged 60+ with a 1:14 nurse-to-patient ratio.

Responsibilities include medication administration, phlebotomy, IV starts, EKGs, charting in EPIC, and collaboration with LNA/CNA/PCT staff. 12-hour day shifts, with weekends and holidays required; housing and travel reimbursements available.

Qualifications

• Active LPN license required if applicable
• Minimum 1 year of clinical experience
• EPIC EMR experience within the past 2 years required and nonnegotiable
• IV certification preferred
• Dementia capable care certification preferred
• Reliable personal transportation required due to lack of public transit access, with all-season or snow tires recommended November through April

Responsibilities

• Post-acute skilled nursing rehab unit within a long-term care nursing and rehabilitation facility
• Patient population aged 60+ requiring skilled nursing rehab care
• Nurse to patient ratio of 1:14 with ancillary staff support including 3 LNA/CNA/PCT per shift and a unit secretary on day shift
• Documentation and medication administration managed through EPIC
• Uses IV pumps and performs phlebotomy, EKGs, transports, nebulizer to inhaler conversions, and IV starts
• 12-hour day shifts scheduled Monday through Sunday with varied scheduling and no guaranteed days off
• Minimum every other weekend requirement with schedule adjusted to meet unit needs
• Must be able to work all holidays
• Occasional/as-needed floating to long-term care and memory care units
