Job summary

ConnectedHealth is seeking a licensed practical nurse for a 13-week assignment in or near Hendricks, Minnesota. This travel-friendly role offers competitive pay and comprehensive benefits while delivering quality care in a long-term care setting.

You will provide bedside care, administer medications, document care in EMR, and collaborate with an interdisciplinary team to develop individualized care plans. An MN LPN license with CPR/IV/BLS certifications and 1 year LTC experience are required;

Qualifications

• Valid LPN license in Minnesota required.
• CPR, IV, and BLS certifications required.
• Minimum 1 year LPN experience in long-term care or similar setting.
• Ability to work evening shifts.
• Strong communication and basic computer skills.
• Attention to detail and adaptability.

Responsibilities

• Provide bedside care and administer medications to residents in accordance with state and facility regulations
• Assess residents' health conditions and report significant changes to the supervising healthcare team
• Maintain accurate documentation of patient care, treatments, and observations in the electronic medical record
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Ensure compliance with safety protocols and quality standards to promote a safe environment for residents and staff
