Qualifications

• Active LPN/LVN license in the state of practice (California required).
• BLS Health Care Provider certification.
• Minimum of two years of nursing experience.
• Education: High School Diploma or GED required; Bachelor's degree or equivalent preferred.
• Weekend availability is required for this travel assignment.
• EPIC experience and hospice experience preferred.

Responsibilities

• Provide general nursing care to assigned patients under supervision of an RN or MD, according to established policy.
• Participate in planning for and provision of patient care; assist in developing a prioritized interdisciplinary plan of care.
• Perform clinical procedures within scope (e.g., phlebotomy, injections, drug screens) and administer medications.
• Maintain aseptic technique for dressings, wound care, and related treatments; monitor and report changes in patient condition.
• Coordinate rounding and visits, assist in discharge planning, and deliver patient education per plan of care.
• Restock and reorder supplies as needed; maintain neat, orderly clinical areas.
• On-call duties may be assigned based on location and agency needs.
