Qualifications

• Licensed Vocational Nurse (LVN) license in California is required.
• Current BLS Health Care Provider certification required.
• At least two years of recent field experience with active nursing license.
• High School Diploma or GED required; Bachelor's degree or higher preferred.
• EPIC EMR experience required; Arrhythmia certification preferred.

Responsibilities

• Provide nursing care to assigned patients under supervision of an RN or MD.
• Perform routine visits and develop an interdisciplinary care plan for each patient.
• Administer medications and perform clinical procedures with monitoring of patient responses.
• Coordinate care plan and assist with discharge planning and patient/family education.
• Restock supplies and maintain clean, organized patient areas; supervise ancillary staff as needed.
• Participate in on‑call duties as required by location and needs.

Responsibilities

• Provide nursing care to assigned patients under supervision of an RN or MD, following established policy and the nursing process.
• Perform routine visits and participate in the development of a prioritized interdisciplinary plan of care for each patient.
• Administer medications and perform clinical procedures (e.g., phlebotomy, injections, drug screens) while monitoring patient responses.
• Implement and coordinate the care plan; assist with discharge planning and patient/family education.
• Restock supplies as needed and maintain neat, clean, and organized patient areas; provide functional direction to ancillary personnel.
• Adhere to aseptic technique and infection control standards; participate in on‑call duties as required by location and operational needs.

Requirements

• Licensure: Licensed Vocational Nurse (LVN) licensure in the state of practice (California) is required.
• Certifications: Current Basic Life Support (BLS) Health Care Provider certification required.
• Experience: At least two years of recent field experience; current nursing license per the state of assignment.
• Education: High School Diploma or GED required; Bachelor's degree or higher preferred.
• Specific qualifications: EPIC electronic medical record experience or recent hospice experience required; Arrhythmia certification preferred.
• Driving/availability: Valid driver’s license preferred; must be at least 21 years old; ability to work weekends and three days during the week as needed.
