Job summary

Healthforce Healthcare LLC is seeking a Licensed Practical Nurse (LPN) for a long-term care assignment in Silver City, NM. The travel position runs for 7 weeks starting 10/05/2026, with 36 guaranteed hours per week on a rotating night schedule (3x12 hours, 18:00–06:30).

Responsibilities include direct patient care, vitals, medication administration under supervision, and collaboration with the care team in a LTC nursing home environment. Active NM LPN license and LTC experience preferred.

Qualifications

• Active Licensed Practical Nurse license in New Mexico (or eligibility).
• Completion of an approved LPN program (diploma or degree).
• Prior long-term care experience preferred.
• Ability to work night shifts (18:00–06:30) and a 3x12-hour schedule.
• Strong clinical assessment, documentation, and communication skills.
• Local assignment availability as indicated in the job notes.

Responsibilities

• Provide direct patient care within the scope of practice for an LPN in a long-term care setting.
• Monitor residents’ conditions, document vitals, administer medications under supervision, and respond to changes in status.
• Assist with activities of daily living and support resident mobility, comfort, and safety.
• Perform basic wound care, skin integrity checks, and infection control practices as directed.
• Document care accurately, update care plans, and communicate changes to the supervising RN.
• Collaborate with the interdisciplinary team, including nursing, therapy, and dietary staff, and report concerns promptly.
