Job summary

Healthforce is seeking a Licensed Practical Nurse (LPN) for a travel assignment in Elkins, West Virginia. Start date 09/14/2026 for 11 weeks with 24 guaranteed hours per week. The shift is 12 hours (07:00-19:00) three days a week, with every other weekend on a two-week rotation.

Setting: Long Term Care in a nursing home. Responsibilities include direct nursing care under an RN, medication administration, vital sign monitoring, ADL assistance, and proper charting.

Qualifications

Active LPN license in West Virginia.
Ability to work 12-hour shifts in a long-term care environment.
Strong communication and teamwork skills.
Eligibility to complete required credentialing and orientation processes.
Willingness to participate in the assigned weekend rotation and fixed shift schedule.

Responsibilities

Provide direct nursing care to residents in a long-term care setting under the supervision of a registered nurse.
Administer prescribed medications and treatments according to policy and clinician orders.
Monitor vital signs, assess health changes, and promptly report to the supervising RN.
Assist residents with activities of daily living, promoting safety, dignity, and independence.
Document care provided accurately in the chart and ensure proper handoffs at shift change.
Collaborate with the interdisciplinary team and participate in care planning and routine quality tasks.
