Qualifications

• Requires current RN licensure in state of clinical office location.
• Requires five (5) years of clinical nursing experience; hospice experience preferred.
• In-depth knowledge of hospice regulations and ability to provide case management and documentation.

Responsibilities

• Provide coordination of referrals and coordination of patient care.
• Oversee all patient care services and personnel.
• Ensure patient needs are continually assessed and care plans are developed, implemented, and updated.
• Provide documentation for patient care and staff performance.
