Job summary

Liberty Hospice in Durham, NC seeks an experienced Hospice Clinical Manager – Travel to cover NC statewide. The role leads patient care coordination, oversees clinical staff, and ensures individualized care plans are developed and updated with thorough documentation.

The ideal candidate holds an active NC RN license, has at least five years of clinical nursing experience (hospice preferred), and strong communication and computer skills. Travel with overnight stays is required.

Qualifications

Requires current RN licensure within the state of North Carolina.
Five years of clinical nursing experience; hospice experience preferred.
Knowledge of hospice regulations and documentation requirements.
Ability to organize work, provide case management, and document care.
Infection control and safety knowledge; quality improvement orientation.
Strong English communication and critical thinking skills.
Proficiency with computers (Word, Excel, Windows).
Minimum 12 hours of continuing education annually; CPR certification.
Valid driver's license; willingness to travel with overnight stays.

Responsibilities

Coordinate referrals and patient care.
Provide oversight of patient care services and personnel.
Ensure patient needs are continually assessed.
Develop, implement, and update individualized care plans.
Document patient care and staff performance appropriately.
Assist Administrator as necessary.
Provide direct patient care as needed, including case management.
When Administrator is unavailable, assume responsibilities as designated in writing.
Accept interim Clinical Manager assignments across Liberty locations.
