Qualifications

• Requires five (5) years of clinical nursing experience. Prefer hospice experience.
• Requires in-depth knowledge of hospice regulations.
• Must be able to organize work, provide case management, home/hospice services, and timely documentation.

Responsibilities

• Coordinate referrals and patient care.
• Oversee patient care services and personnel.
• Ensure ongoing assessment of patient needs.
• Develop, implement and update individualized care plans.
• Document patient care and staff performance.
• Assist Administrator as necessary.
• Provide direct patient care as needed, including case management.
• Travel as interim Clinical Manager when needed.
