Qualifications

• One year of RN experience with at least 6 months in Home Health.
• Proficient in OASIS documentation.
• Valid RN license per state regulations.
• BLS certification.

Responsibilities

• Assess patient conditions and develop individualized care plans.
• Administer medications, treatments, and wound care.
• Educate patients and caregivers on medications and self-care.
• Monitor patient changes and adjust care plans as needed.
• Coordinate with physicians and therapists for smooth care transitions.
• Document care and progress in compliance with standards.
• Maintain infection control and safety in home care.
• Support families with guidance and emotional support.
