Job summary

Fusion Medical Staffing seeking a Home Health RN for a 13-week travel assignment in St. Louis, MO. You will deliver home-based nursing care, develop care plans, and educate patients and caregivers while collaborating with a clinical team.

Qualifications

• One year of RN experience with at least six months in Home Health.
• Proficient in OASIS documentation and nursing documentation.
• Valid RN license compliant with state regulations.
• BLS certification (AHA/ARC) required.

Responsibilities

• Assess patient conditions and develop individualized care plans.
• Administer medications, treatments, and wound care; monitor responses.
• Educate patients and caregivers on medications and disease management.
• Monitor for changes and adjust plans as needed.
• Coordinate with physicians, therapists and other staff for seamless care.
• Document care, interventions and progress per standards.
• Maintain infection control and safety in home care settings.
• Support families with guidance and emotional support.
• Communicate with case managers for coordinated care.
• Complete charting and meet visit/productivity goals.
• Perform other duties within scope of practice.
