Job summary

Sweetwater Care is seeking an experienced Home Health RN to visit patients in their homes, assess conditions, monitor meds and vital signs, and adjust care plans accordingly.

You will provide wound care, administer medications, and educate families on care procedures, while coordinating with physicians and therapists to ensure comprehensive, compassionate patient support.

Qualifications

• Must have strong interpersonal skills and the ability to build trust.
• Must be able to make independent decisions and follow patient needs.
• Must respond swiftly in emergencies and adapt to changing conditions.
• Flexibility to work in diverse home environments and schedules.
• Proficient in wound care, tube feedings, injections and IV setup.

Home Health RN Duties and Responsibilities:

• Complete evaluation tasks, including reviewing medication and vital signs.
• Administer Physician-prescribed medication.
• Dress or redress wounds and assess the healing progress.
• Provide education to patients and families on proper home health care procedures and strategies.

• Initiative so they can make independent decisions when required and accept direction and requests from patients.
• Ability to act swiftly in emergencies and work efficiently
• Flexibility and patience to work in a variety of living environments
• Knowledge of clinical skills including changing bandages, giving tube feedings, administering shots and setting up and monitoring IVs
