Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel job in Paris, Texas. This 13-week assignment requires a PT license, BLS, and at least 1 year of home health experience. You will provide patient-centered PT services across Paris and surrounding areas with 40 hours per week in 8-hour shifts.

The contract pays $2,134–$2,315 per week, with travel time factored into productivity and comprehensive benefits supported by SHC.

Formación

• Active Physical Therapist license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and health clearance.

Responsabilidades

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.
