Supplemental Health Care is seeking a travel Home Health Physical Therapist to Arlington, Texas for a 13-week assignment. Provider will cover 40 hours per week, with 6 visits daily, and travel between patient homes as needed. Licensure, BLS, and home health experience are required to deliver compliant, patient-centered care.

The role emphasizes PT evaluations, OASIS-E familiarity, and collaboration with an interdisciplinary team to ensure quality outcomes under Medicare guidelines.

Qualifications

• Active, unrestricted Physical Therapist license in the assignment state
• Current BLS certification (AHA)
• 1+ year of recent home health experience
• Experience conducting PT evaluations and developing therapy plans
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
• Reliable transportation and ability to travel between patient homes

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
• Conduct gait and balance evaluations, home safety assessments, and mobility training
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care
• Document all care in compliance with Medicare, OASIS-E and agency standards
