Qualifications

• Active Physical Therapist license
• Licensed in Ohio or compact eligible
• Two years of Home Health/Hospice exp.
• Active BLS certificate

Responsibilities

• Provide home health and hospice PT services.
• Adhere to travel assignment schedules and 40 hours/week.
• Document care plans and progress in a timely manner.
• Coordinate with care team and supervisors.
