Supplemental Health Care is seeking a traveling Home Health Physical Therapist for a Springfield, Ohio assignment. You will deliver patient-centered PT care across homes with 40-hour weeks and 8-hour day shifts, within a 13-week contract.

Required: active PT license in the state, current BLS, and 1+ year home health experience. Knowledge of Medicare/OASIS-E and EMRs like Homecare Homebase, NetSmart, or Epic is essential.

Qualifications

• Active PT license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of home health experience.
• Experience with PT evaluations and developing plans.
• Familiar with Medicare/OASIS-E documentation and guidelines.
• Experience with EMRs such as Homecare Homebase, NetSmart or Epic.

Responsibilities

• Perform initial and ongoing PT assessments and develop plans of care.
• Provide interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait/balance evaluations and home safety assessments.
• Educate patients and caregivers on exercises and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document all care per Medicare, OASIS-E, and agency standards.

Qualifications

• Active, unrestricted Physical Therapist (PT) license in the state of assignment
• Current BLS certification (AHA)
• 1+ year of recent home health experience (within the last 2 years)
• Experience conducting PT evaluations and developing therapy plans
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
• Reliable transportation with valid auto insurance and ability to travel between patient homes

Home Health Physical Therapist Contract Details

• $1,864 - $2,038 per week*
• 13-week contract
• Paid by the hour - not by the visit
• 5–6 visits per day (45–90 minutes each, based on complexity)
• Travel time factored into productivity expectations

Key Responsibilities

• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
• Conduct gait and balance evaluations, home safety assessments, and mobility training
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
• Document all care in compliance with Medicare, OASIS-E, and agency standards
