American Traveler, LLC. is seeking a licensed Physical Therapist to provide home health services across multiple Ohio ZIP codes. This full-time day-shift role runs 8:00 AM–5:00 PM and serves communities within listed areas.

Responsibilities include delivering skilled therapy in patients' homes, coordinating with care teams, and documenting care. Licensure in Ohio is required before starting; HomeCare HomeBase experience is a plus but not mandatory. Competitive benefits are included.

Qualifications

• Valid state PT license required prior to starting; pending licenses not accepted.
• License required if applicable
• Experience with HomeCare HomeBase charting system preferred but not required.

Responsibilities

• Provide home health physical therapy services in patients' homes across multiple Ohio ZIP codes.
• Perform patient assessments, develop and implement treatment plans, and document progress.
• Coordinate with care teams and manage scheduling and documentation per policy.
