Description

American Traveler is seeking a Home Health Physical Therapist with 1+ year of recent experience and HCHB EMR proficiency to cover a geriatric caseload in a home health territory.

Details

• Home Health setting covering a territory within a 60-mile radius of the hub office
• Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
• Average caseload of 5-7 patients per day
• Patient population consists of geriatric home health cases
• Uses HCHB (Homecare Homebase) EMR system
• On-call rotation required approximately every 3rd to 4th weekend
• Traveler must remain flexible with assigned territory

Requirements

• Minimum 1 year of recent Physical Therapy experience required
• BLS certification required
• OASIS experience required for completing Start of Care (SOC) documentation
• Proficiency in HCHB EMR required
• License required if applicable
• Active driver's license and personal transportation required
• Must provide timely documentation and communication

Additional Information

• Position covers an FMLA leave starting in February
• Scrubs required in any color; jeans not permitted
• No cell phone provided; iPad provided for work use
• Mileage reimbursed at the IRS standard rate
• Monday orientation required, including completion of billable modules
• Hospice/Home Health checklist completion is mandatory
• RTO requests must be submitted at the time of application
• First-time travelers are not accepted for this position
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, though each traveler will be assigned their own territory and must have personal transportation
