Description

American Traveler is seeking a Physical Therapist for a Home Health position requiring 2+ years of home health experience and HCHB/Oasis charting proficiency.

Details

• Home health setting covering White, Cumberland, and Van Buren Counties within a 50-75 mile radius
• Day shift, 5x8 hour schedule from 8:00 AM to 5:00 PM
• Patient population is geriatric, with visit types including start of care and follow-up visits
• Average caseload of 6-7 patients per day, equating to approximately 3 patient visits
• Productivity requirement of 30 points per week minimum
• No on-call requirements
• No weekend work required unless needed to meet weekly productivity goals
• Uses HCHB and Oasis for charting

Requirements

• Current state license required if applicable
• BLS certification required
• Minimum 2 years of home health experience required
• HCHB and Oasis charting experience required
• Must hold license in hand, pending licenses not accepted
