Job summary

American Traveler is seeking a Home Health Physical Therapist with 2+ years of home health experience for a 13-week travel assignment in California. You will provide therapy in a home health setting with a 4-5 patient caseload per day, documenting within 1 day using the HCHB EMR.

Schedule is Mon-Fri, 8am-5pm, with mileage reimbursement and housing options. Requirements include active CA PT license or compact license, current BLS, valid driver's license, and two professional references.

Qualifications

• Minimum 2 years of PT experience, with prior home health experience required.
• Current BLS certification required.
• CA PT license required if applicable; active state or compact license required at submission.
• Valid driver's license and car insurance required.
• Active state or compact license required at time of submission; pending licenses not accepted.

Responsibilities

• Home Health care setting serving patients of all ages with multiple comorbidities
• Caseload of 4-5 patients per 8-hour day
• Documentation must be completed within 1 day of patient visit
• Uses HCHB (HomeCare HomeBase) EMR
• Schedule is Monday-Friday, 8am-5pm, 5x8-hour day shifts
• Support from PTAs/COTAs and 24/7 manager availability
• Mileage reimbursed per GSA rate
• Responsibilities include standard physical therapy treatment and timely clinical documentation
• All MyJourney training modules must be completed prior to arrival; travelers may be delayed if incomplete
• Copy of driver's license required for consideration
• Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and rehire eligibility
• Facility may cancel up to 4 shifts per 13-week assignment
