TalentBurst, Inc is seeking a Travel Home Health Physical Therapy Assistant for a 13-week assignment in Hesperia, California. The role is 40 hours per week, days shift, working in home health with geriatric patients and an average of 5-7 patient visits daily.

Requirements include 1 year of recent Home Health experience, BLS certification, CA license in hand, and use of EMR (HCHB EXP REQ). Mileage reimbursed and travel rate apply within a 60-mile radius from the hub.

Qualifications

• 1 year minimum Home Health/PTA experience.
• BLS certification required.
• CA license in hand.
• EMR experience with HCHB EXP REQ.

Responsabilités

• Provide PTA services in a home health setting with geriatric patients.
• Assess and treat patients; develop and monitor therapy plans.
• Educate patients and families on exercises and safety.
• Document progress and communicate with the care team per guidelines.
