Job summary

Supplemental Health Care is seeking a Home Health Physical Therapist to provide patient-centered care in Everett, WA. The role is a 13-week contract with 40 hours per week, dayshift, travel between homes, and a focus on Medicare guidelines and OASIS-E documentation.

Responsibilities include assessments, care planning, mobility training, and collaboration with RNs and physicians to ensure coordinated care. SHC offers comprehensive benefits and ongoing support.

Qualifications

• Active PT license in the state of assignment (WA).
• Current BLS certification (AHA).
• 1+ year of home health experience in the last 2 years.

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations and home safety assessments.
