Job summary

Your Health Organization is seeking a Clinical Coordinator to support Macon patients with essential chart prep, scheduling, and care coordination. The role travels between ALF, SNF and IL facilities, with daily travel required and a standard 8‑hour schedule (8:00AM‑5:00PM, Monday‑Friday).

The coordinator will use Navina for chart updates, coordinate with the care team, and communicate with patients through multiple channels to ensure seamless care delivery.

Qualifications

• Must be CMA or licensed nurse and licensed in the state of employment.
• Minimum of 1 year experience in a healthcare setting preferred.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent required.
• Strong interpersonal and communication skills; team player with good judgment.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all documentation, test results, and diagnoses are accessible for providers; update Navina as needed.
• Maintain patient confidentiality and ensure consent forms and alignment paperwork are in charts.
• Reconcile medications.
• Coordinate scheduling of patient appointments with the care team and providers.
• Interact with patients via email, portal, phone, text, or in person to gather info and provide pre-appointment instructions.
• Document and log care management activities.
