Job summary

Your Health in Myrtle Beach is seeking a Clinical Coordinator to manage clinical tasks for patients across ALF, SNF, and other facilities. The role emphasizes chart preparation, scheduling visits, logging care management activities, and supporting providers and patients in a fast-paced value-based care model.

The position is full-time, 8 hours per day, Monday to Friday, with daily travel between facilities.

Qualifications

• Must be CMA or licensed nurse with active state license.
• Minimum 1 year healthcare experience preferred.
• Current CPR certificate.
• Strong interpersonal and communication skills.

Responsibilities

• Review and organize patient medical records for visits.
• Ensure documentation, test results, and diagnoses are accessible for the provider.
• Maintain confidentiality and ensure consent forms are in the chart.
• Reconcile medications.
• Schedule and coordinate patient appointments with the care team.
• Interact with patients via email, portal, phone, text, or in person to gather info and provide pre-visit instructions.
• Document and log care management activities.
