Job summary
Your Health Organization in Myrtle Beach seeks a Clinical Coordinator to support clinical needs for our patients across facilities. The role involves chart prep, scheduling visits, and logging chronic care management activities with daily travel between ALF, SNF and IL facilities in the Myrtle Beach area.
Must have CMA or nursing license, CPR, and strong communication skills; travel and reliable transportation required.
Qualifications
- Must be a CMA or licensed nurse (LPN or RN) with license in good standing.
- Minimum of 1 year in a healthcare setting preferred.
- Must hold current CPR certificate.
Responsibilities
- Review and organize patient medical records for visits.
- Ensure documentation, test results, diagnoses are accessible for providers; use Navina to update records.
- Ensure confidentiality and privacy compliance; verify active consent forms.
- Reconcile medications.
- Schedule patient appointments with providers and care team.
- Communicate with patients via email, portal, phone, text, in person.
- Utilize communication tools such as phone, email, AthenaText, MS Teams.
- Document and log care management activities.
