Job summary

• Document evaluations, plans, progress notes and discharge summaries per policy.
• Collaborate with PTs, nursing and other disciplines to optimize outcomes.
• Educate residents and families on home exercise programs and strategies.
• Monitor safety, equipment use, and infection control.

Responsibilities

• Conduct OT evaluations and participate in the development of individualized treatment plans for residents in a geriatric, skilled nursing setting.
• Implement and adjust therapy activities to promote functional independence in activities of daily living and mobility.
• Document evaluations, treatment plans, progress notes, and discharge summaries in accordance with facility policies and regulatory requirements.
• Collaborate with physical therapists, nursing staff, and other members of the interdisciplinary team to optimize resident outcomes.
• Provide education and training to residents and families on home exercise programs and strategies to support continued progress.
• Monitor resident safety, use of adaptive equipment, and adherence to infection control and safety protocols.

Requirements

• Current Certified Occupational Therapy Assistant (COTA) certification
• Graduate of an accredited OTA program
• Eligibility for state licensure or credentialing as required (e.g., Maine) and ability to obtain licensure if applicable
• Experience or interest in geriatric rehabilitation and skilled nursing facility care; ability to work in a team-based environment
• Proficient in clinical documentation and use of electronic health records; strong communication skills
• BLS certification preferred
