Qualifications

Responsibilities

• Conduct OT evaluations and help develop individualized treatment plans for geriatric residents in a skilled nursing setting
• Implement and adjust therapy activities to promote independence in activities of daily living and mobility
• Document evaluations, treatment plans, progress notes, and discharge summaries per facility policies
• Collaborate with physical therapists, nursing staff, and the inter-disciplinary team to optimize resident outcomes
• Educate residents and families on home exercise programs and strategies to support progress
• Monitor safety, use of adaptive equipment, and adherence to infection control procedures

Responsibilities

• Monitor resident safety, use of adaptive equipment, and adherence to infection control and safety protocols

Requirements

• Current Certified Occupational Therapy Assistant (COTA) certification
• Graduate of an accredited OTA program
• Eligibility for state licensure or credentialing as required (e.g., Maine) and ability to obtain licensure if applicable
• Experience or interest in geriatric rehabilitation and skilled nursing facility care; ability to work in a team-based environment
• Proficient in clinical documentation and use of electronic health records; strong communication skills
• BLS certification preferred
