Job summary
Connecticut, LLC is looking for an Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) for Skilled Nursing Facilities in Hartford. This role offers a flexible work schedule and the opportunity to provide essential care to a complex patient population.

Qualifications
- Masters or doctorate prepared Advanced Practice Registered Nurse (APRN) or certified Physician Assistant (PA).
- Registered Nurse and Advanced Practice Registered Nurse licenses required.
- Experience with elderly patients and electronic health records (EHR).
Responsibilities
- Provide medical care to patients in short and long-term care settings.
- Analyze patient medical history to determine the best care.
- Order and interpret diagnostic tests to assess patient needs.
- Prescribe medications and treatment regimens.
- Record findings and plans in medical records.
- Coordinate with care planning teams for comprehensive care.
- Communicate with patients and families regarding care plans.
