Job summary

Physicians Alliance of Connecticut (PACT, LLC) has an opportunity for an Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) to work Part Time in Skilled Nursing Facilities in the Branford area. You will travel to contracted facilities and provide medical care to a complex patient population in rehab and long-term care settings.

The role offers flexible scheduling, autonomous work, and a pay-for-performance environment.

Qualifications

The Advanced Practice Registered Nurse is masters or doctorate prepared.
Certified Nationally as a Nurse Practitioner through AANP or ANCC.
The Nurse Practitioner must have possession of a valid CT RN and APRN license and be certified as a Nurse Practitioner by an approved certifying body.
DEA and CT controlled substance license.
Experience working with elderly patient population.
Experience working with EHR.

Responsibilities

Provide medical care to a complex patient population in the short term rehab and long term care setting.
Analyze patient’s medical history, medication allergies, physical condition and examination to determine best care.
Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs.
Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate.
Record in medical records the physical findings, plans, and prognosis within appropriate time frame.
Coordinate with the nursing facility care planning team in developing comprehensive care plan.
Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as needed.
Follow established departmental policies, procedures, and objectives, continuous quality improvement objectives, safety, environmental, and infection control standards.
