Job summary

Connecticut, LLC is seeking an Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) for skilled nursing facilities in Stamford. This full-time on-site role offers a competitive salary range of $110,000.00 to $200,000.00 annually, with performance-based bonuses available.

The ideal candidate will work autonomously, provide comprehensive medical care, and coordinate treatment plans with nursing facility teams. Experience with elderly patients and EHR is highly valued.

Qualifications

• Advanced Practice Registered Nurse must have a master's or doctorate prepared.
• National certification as a Nurse Practitioner is required.
• Experience with electronic health records (EHR) is beneficial.

Responsibilities

• Provide medical care to complex patient populations in the rehab and long-term care settings.
• Order, interpret, and evaluate diagnostic tests as needed.
• Communicate with patients and family regarding care plans and prognosis.
