Job summary

Hearts With Hope Foundation seeks a Youth Family Support Specialist to work closely with youth in foster care and their families, focusing on permanency and family connections. The role provides guidance, service coordination, and training to strengthen relationships and empower caregivers to meet children's needs.

Qualifications

• Provide guidance, support, service coordination and training to families.
• Build trust-based relationships with youth and families to support permanency.
• Advocate and coordinate services with schools, social service agencies and mental health providers.

Responsibilities

• Conduct comprehensive and periodic assessments of youth and family needs to determine eligibility and plan.
• Develop search and engagement strategies to find permanency options.
• Identify significant adults and maintain family finding and engagement throughout work with youth.
• Represent families in treatment meetings to develop CPOS addressing needs across spiritual, physical, educational, emotional and social aspects.
• Provide regular check-ins with the family (at least weekly) and help parents understand their children’s needs.
• Ensure youth progress toward safe and lasting family relationships and living situations.
• Provide case management and coordinate connections with schools, social services and mental health providers.
• Coordinate referrals to services and related agencies to meet CPOS needs.
• Learn and maintain knowledge of community resources and sustain partnerships.
• Assist families in setting functional, family-centered outcomes and advocate for flexible supports.
• Promote trauma-informed parenting practices and coach effective behavior management.
• Monitor progress on youth outcomes related to social-emotional skills and learning.
• Work with families in an empathic, professional and objective manner.
• Collaborate with other service providers to coordinate services and maintain contacts.
• Participate in community liaison activities and family outreach at events and meetings.
• Expose families to programs and trainings, updating knowledge as needed.
• Ensure privacy and confidentiality of all family records.
• Coordinate Youth Advisory Board meetings to enhance youth engagement.
• Maintain a flexible schedule including evenings and weekends.
• Perform other duties as assigned.
