Job summary

YFA (Youth and Family Advocates) Polk County Community Counselor position invites professionals to provide trauma-informed counseling to youth and families, conducting assessments, developing treatment plans, and delivering individual and group therapy under supervision.

Responsibilities include intake, case management, collaboration with schools, and documentation to meet regulatory requirements; commitment to safety and culturally sensitive service delivery is essential.

Qualifications

• Master's degree in social work, mental health, psychology, or other human services degree and 0-3 years’ post-master’s degree clinical/counseling experience.
• Knowledge and understanding of trauma-informed principles and practices, including creating a safe environment and empowering individuals.
• Ability to communicate effectively, both verbally and in writing; maintain working relationships with internal and external contacts.
• Proficiency in documenting client interactions and maintaining accurate records.

Responsibilities

• Conduct assessments, develop treatment plans, and complete psychosocial evaluations to meet contract and regulatory requirements.
• Provide counseling and support to youth and their families to promote education and pro-social behavior.
• Determine eligibility, complete intake/evaluation, coordinate referrals, and monitor school attendance.
• Conduct individual and group counseling sessions; schedule sessions at client-preferred locations.
• Maintain regular communication with schools and agency contacts to coordinate services.
