Job summary

New Directions Youth & Family Services, Inc. seeks a Children’s Care Manager to deliver Health Home Care Management for youth ages 0–21. You will coordinate services, engage families, and link them with community resources using a trauma-informed, strength-based approach.

The role focuses on planning, care coordination, and ensuring continuity of care across transitions, with ongoing staff training and supervision.

Qualifications

• Bachelor’s degree in a human services field and two years of youth work experience.
• Master’s degree in a human services field and one year of related experience.
• Valid driver’s license and adequate auto insurance.

Responsibilities

• Facilitate collaboration among providers via MDT meetings.
• Engage youth and families in developing individualized Plans of Care.
• Link families with medical, behavioral, and community resources.
• Maintain a caseload of 12–40 youth and conduct monthly contacts.
• Provide crisis and safety planning and 24/7 on-call support.
• Ensure timely documentation and Medicaid/audit compliance.
