GLIDE is seeking a part-time Spiritual Care Navigator in San Francisco to provide trauma-informed, holistic spiritual care for individuals facing serious illness or end-of-life transitions. The role includes bedside visits, outreach, and faith-based programs, with a focus on dignity, healing, and connection for patients and families.

The ideal candidate has 2–3 years in faith-based care, grief support, and group facilitation, and can work independently within a multidisciplinary team.

Formación

• Minimum of 2-3 years of experience in faith-based organizing, pastoral care, spiritual counseling, or a related field.
• Experience in grief counseling and emotional support, especially in palliative and end-of-life contexts.
• Comfort and experience in facilitating group programs, including creative arts workshops, reflective practices, and spiritual rituals.
• Ability to work independently and as part of a multidisciplinary team.
• Flexible and adaptable in meeting the needs of diverse health and life circumstances.
• Regulatory compliance requirements completed.
• Strong interpersonal and communication skills, with the ability to build trust with patients, families, and team members.
• Knowledge of and sensitivity to various religious and spiritual traditions.
• Ability to provide compassionate support in emotionally charged and challenging situations.
• Excellent organizational skills and ability to manage multiple responsibilities.

Responsabilidades

• Provide spiritual care, counseling, prayer, bedside visits, and emotional support to patients, families, and caregivers.
• Respect and honor diverse religious and spiritual beliefs.
• Support individuals through serious illness, end-of-life planning, grief and bereavement conversations.
• Conduct outreach in churches, hospitals, shelters, and community settings.
• Facilitate referrals to hospice, healthcare, behavioral health, housing, and senior services.
• Assist with advance healthcare directives and goals-of-care discussions.
• Develop and facilitate support groups and faith-based education programs.
• Collaborate with interdisciplinary teams to coordinate holistic care.
