Qualifications

• Graduation from an accredited nursing program and VT licensure as RN or LPN.
• Valid VT driver’s license or reliable transportation.
• Background check and motor vehicle record check required.

Responsibilities

• Coordinate admissions and discharges and assess healthcare needs.
• Develop individualized health plans and provide health education.
• Administer prescribed medications and maintain MAR/eMAR records.
• Document timely patient information and ensure regulatory compliance.
• Deliver an eight‑week health and wellness curriculum to clients and families.
