Qualifications

• High School Diploma, GED or equivalent, required.
• Valid Driver’s License and acceptable driving record, required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsibilities

• Develop individualized, person-centered service plans and support their implementation.
• Assist with daily living activities, skill development, and personal goal attainment.
• Help individuals access transportation and community resources as needed.
• Implement activities promoting community inclusion, relationship building, personal growth, and valued social roles.
• Maintain a safe, clean, and supportive environment for individuals served.
• Review, complete, and maintain required documentation in a timely manner.
