Job summary

Heartland Family Service in Council Bluffs, IA, seeks a Psychiatric Rehabilitation Practitioner to deliver Intensive Psychiatric Rehabilitation (IPR) services to adults with mental illness using a structured, recovery-oriented approach.

The role requires creativity, initiative, and the ability to run engaging groups, conduct outreach activities, and monitor progress while meeting program revenue goals. Compensation ranges from $19.02 to $22.68 per hour, with a 37.5 hour work week.

Qualifications

• Bachelor’s degree and one year of experience working with adults living with mental illness.
• 60 contact hours of Intensive Psychiatric Rehabilitation Service training after hire.
• Valid Driver's License and acceptable driving record.

Responsibilities

• Provide all aspects of IPR including triage, readiness assessment, readiness development, goal choosing, goal achieving and goal keeping for individuals.
• Create, plan, prepare, and facilitate daily and weekly IPR groups that are purposeful and engaging.
• Plan and facilitate community-based and social outings as active rehabilitation opportunities.
• Assist clients in identifying and maintaining healthy personal and social boundaries.
• Complete documentation using MyEvolv and meet program standards.
• Meet monthly IPR revenue and productivity goals with an active caseload.
• Attend supervision meetings and staff meetings.
• Communicate with multidisciplinary team members and supports.
• Provide crisis intervention and planning to help clients access services.
• Manage scheduling for clients and ensure participation in IPR services.
• Develop individualized case plans with clients and monitor progress.
• Communicate information to other agencies as appropriate.
• Work independently and as part of a team; meet expectations.
• Be punctual and dependable; adhere to regulations.
• Create and share a dynamic self-care plan.
• Help connect the agency with the community to further fundraising efforts.
