Qualifications

• Master's degree in social work or counseling.
• LCSWs, LCPC required; LADC preferred.
• Valid state driver's license.

Responsibilities

• Provide customer service within established deadlines, consistent with organizational values, standards, and policies.
• Demonstrate leadership in Case-Coordination.
• Complete timely, professionally written, documentation.
• Manage insurance authorizations in a fiscally responsible manner.
• Use quality Collaborative Documentation to maintain insurance required documentation.
• Coordinate clinical appointments.
• Use the Organization's established tools and procedures, to formulate a Comprehensive Assessment.
• Create clinical interventions in an Individualized Service Treatment Plan reflecting best-practice and measurable outcomes.
• Demonstrate Trauma-Informed Treatment.
• Provide Integrated Care by including the client's Natural Supports and Providers.
