Job summary

ICL is seeking an Occupational Therapist to join its Health Connect Program within Supported Housing in Brooklyn, NY. You will conduct health assessments, develop individualized care plans, and support clients in achieving recovery goals through trauma‑informed, person‑centered care approaches.

Work closely with a multidisciplinary team to promote independence, community integration, and access to healthcare resources in supportive housing settings. NY OT license required.

Qualifications

• Master’s degree in Occupational Therapy from an accredited institution.
• Experience with Serious Mental Illness or substance use disorders preferred.
• Experience in supportive housing or community-based programs preferred.
• Knowledge of trauma-informed and person-centered care approaches.
• Understanding of chronic disease management and preventive healthcare.

Responsibilities

• Conduct comprehensive health assessments addressing psychiatric and medical needs.
• Develop individualized care plans to promote independence and community integration.
• Collaborate with multidisciplinary team to provide holistic care.
• Provide health education and wellness training to clients and staff.
• Maintain accurate documentation in the Electronic Health Record.
