Third Street Family Health Services seeks a Justice-Involved Behavioral Health Clinician to assess, counsel, and support justice-involved individuals as they transition back into the community. You will collaborate with courts, correctional facilities, and community agencies to connect patients with behavioral health services and social supports.

Qualifikationen

• Bachelor's or Master's degree in social work, counseling, psychology, or a related behavioral health discipline.
• Current State of Ohio licensure (e.g., CT, LSW, LPC, LISW, LPCC, LISW-S, LPCC-S).
• Experience providing behavioral health counseling, psychotherapy, assessment, and/or crisis intervention.

Aufgaben

• Conduct behavioral health assessments and provide counseling and interventions for justice-involved individuals.
• Provide jail outreach and community-based support to connect with ongoing services.
• Support transitions from correctional settings through care coordination and treatment planning.
• Collaborate with courts, correctional facilities, probation/parole, and community agencies.
• Assist patients in accessing housing, substance use treatment, primary care, case management, transportation, and social services.
• Respond to crises with risk assessments, safety planning, and referrals.
• Maintain timely documentation of assessments, treatment plans, progress notes, and referrals.
