Job summary

Lad Lake in West Bend, Wisconsin, seeks a compassionate Community Therapist (PT) to join a trauma-informed, family-focused treatment team. The role provides therapy to youth and families in home, telehealth, or office settings across multiple counties.

You will conduct assessments, develop individualized treatment plans, advocate for clients, and document regularly. A Master’s degree and an active license are required, with travel across assigned counties to support communities.

Qualifications

• Master’s degree in social work, Counseling, or related field is required.
• Active licensure (LPC, LCSW, LMFT, or Training License) is required.
• Demonstrated experience working with youth and families in trauma-informed settings.

Responsibilities

• Conduct initial clinical assessments and develop individualized treatment plans.
• Provide therapy to youth and families in-home, virtually, or in-office based on clinical need.
• Advocate for youth and families within community systems.
• Manage and revise crisis response plans as needed.
• Maintain accurate and timely documentation of all clinical services.
• Collaborate with interdisciplinary treatment teams.
